Job Overview
Location: Fairview Southdale Hospital
Hours: 64 hours bi-weekly
Schedule: day/evening/night, every third weekend
This position is responsible for providing medication management, clinical support and consultative services in a complex pharmacy operation. This position partners with key stakeholders to drive patient and family education, medication safety, quality assurance, customer service, process improvement and regulatory compliance. This position may also participate in education of pharmacy students and residents.
Responsibilities
- Provide clinical, formulary and pharmacotherapy expertise and act as a resource to students, staff and interdisciplinary members of the care team.
- Provide clinical pharmacy services to all patients in assigned area using Standard Operating Procedures (SOP). This includes dispensing of medication, completing review for all patients, monitoring, adjusting patient therapies and documenting changes according to department policies and pharmacy regulations.
- Act as resource for pharmaceutical care to patients, partnering with interdisciplinary care team members to ensure appropriateness of medication order and therapy, drug selection, formulary correctness and compliance any other formulary or departmental initiatives as appropriate.
- Accountable for the distribution and/or preparation of medications, sterile products, compound IV mixtures and other extemporaneous orders as appropriate.
- Maintain proficiency in the skills and theory of the practice of pharmacy by reviewing literature, attending in-services, seminars and conferences and actively participating in department clinical meetings and activities.
- Participate in regulatory, quality assurance and process improvement activities as required.
- Meets all regulatory requirements.
- Uses available information technology and other resources to support day-to-day work operations

Requirements
Required Qualifications
- Minnesota Pharmacist License
Preferred Qualifications
- Doctorate of Pharmacy
- 1 year pharmacist experience in complex healthcare environment
- Completion of an ASHP-accredited PGY1 residency program
